North American Medical Nonwovens, Barrier Fabrics and Implantable Textiles: Supply Base and Application Map
Three markets sit under the word textile in medical devices and they have almost nothing in common. Single use protective apparel and drapes is a high volume commodity business reshaped permanently by the 2020 supply shock and the domestic meltblown capacity built in response. Sterilization barrier and wound care is a mid-volume regulated business where switching costs are real. Implantable textiles, meaning vascular grafts, surgical mesh, sutures and heart valve sewing cuffs, is a tiny volume, enormous margin business with the longest qualification cycle in this catalog and a litigation history that shapes how every buyer in it behaves. This lens maps twenty part families across those three markets with a twenty-eight position roster across four tiers. The differentiator is that meltblown capacity is a strategic asset now: North America built substantial domestic meltblown and SMS capacity after 2020, much of it with government support, and that capacity is looking for demand in a market that has partly reverted to imports. A supplier with domestic lines has a Buy American story that matters to federal and hospital system buyers, and idle capacity that matters to its own P and L.

Market Snapshot
Three markets sit under the word textile in medical devices, and a supplier that reads them as one will price the wrong one and staff for the wrong one. They do not share a customer, a plant, a qualification path, a unit of sale or a margin structure. The only thing they share is a fiber.
The first is single use protective apparel and drapes: surgical gowns, isolation gowns, drapes, masks, respirators, shoe covers and the rest of the barrier stack. It is enormous, it is bought by the case on a hospital contract, and it is sold in one language, the AAMI PB70 barrier level. Level 1 through level 4 is the product designation, the catalog number and the entire commercial conversation, and a supplier quoting basis weight and tensile strength has misunderstood what is being bought. Margins are thin and the competitive question is landed cost per gown at a stated level.
The second is sterilization barrier and wound care, and it sits in a genuinely different place. A sterilization wrap is a validated component of a sterile barrier system under ISO 11607, which means a hospital that changes wrap revalidates its sterile processing department. A wound contact layer is a class II device with a clinical claim behind it. Volumes are smaller, specifications are real, and switching costs are meaningful without being prohibitive.
The third is implantable textiles: vascular grafts, surgical mesh, braided and monofilament sutures and heart valve sewing cuffs. It is tiny by volume and extraordinary by margin. A knitted polyester graft fabric is measured in square metres per year and priced in the hundreds of dollars per square metre. It also carries the longest qualification cycle of anything in this catalog and a litigation history that changed the behaviour of everyone in it, because transvaginal mesh produced one of the largest device mass torts in United States history and several resin producers responded by refusing to sell polypropylene into implant applications at all.
Over all three sits the fact that reorganized this industry. In 2020 the supply of surgical masks, respirators and gowns to North America failed, because almost all of it was made somewhere else. The response was a domestic capacity build-out, substantially supported by federal money, that added real meltblown and SMS lines on this continent. The shortage ended. A large share of the volume went back to imports. The capacity did not go away, and what to do about that is the central commercial question on this lens in 2026.
Two regulatory currents run through everything below. The first is the set of domestic content preferences that came out of the same period: Berry Amendment provisions on textiles bought by the Department of Defense, Buy American requirements on federal purchasing, and the stockpile and preparedness commitments that followed. They are the reason domestic capacity has a commercial argument at all. The second is PFAS. The fluorochemical repellent finishes that hold the barrier level on gowns and drapes are precisely the chemistry being restricted, and reformulating to the same AAMI level without fluorine is an active, unfinished engineering problem across the whole protective apparel market.
How the Supply Chain Is Structured
The tier labels on this lens carry a specific meaning that is worth stating before the roster, because the shape here is not the device industry shape the polymers lens describes.
The OEM tier is the device and disposables companies the atlas already carries: Medtronic, Becton Dickinson, Cardinal Health, Baxter, Teleflex, Stryker and Integra LifeSciences are the seven that matter most here. They reach this lens because their positions are written once for the whole industry. But on the commodity half of this report the device OEM is often not the buyer at all, and that is the first thing to understand about the category.
Tier 1 is the branded converters: the companies that turn roll goods into a gown, a drape, a wrap, a dressing or an underpad and put a catalog number and a hospital contract behind it. Owens and Minor manufactures under the Halyard brand it bought in 2018 and is the largest domestic maker of surgical gowns. Medline manufactures its own apparel, wound care, gauze and underpads. Molnlycke built advanced wound care on a soft silicone technology and then made atraumatic removal a purchasing requirement. Cardinal Health does the same work under its own brand and is carried in the OEM tier because it is one. These are the accounts where a nonwoven roll goods decision is actually made.
Tier 2 splits two ways and the split is the report. The nonwoven roll goods producers are Berry Global, Fitesa, Avgol, Freudenberg Performance Materials, Glatfelter, TWE Group, Precision Fabrics Group, Ahlstrom and Alkegen. They sell tonnes of web at cents per square metre against a barrier or a filtration specification. The implantable textile specialists are Secant Group, Biomerix and Confluent Medical. They sell square metres per year at hundreds of dollars per square metre against a 510(k). A supplier in one half has no idea how the other half works, and there is essentially no traffic between them.
Tier 3 is fiber and resin: ExxonMobil and LyondellBasell on polypropylene, Dow on polyethylene and medical silicone, Indorama Ventures on PET resin and fiber, Eastman on copolyester, and then the implant side, DSM Biomedical, Victrex and Zeus, whose products reach this lens as drawn monofilament and fiber rather than as molding resin.
Distribution is Medline, Owens and Minor and Henry Schein, and on this lens it is not a channel detail. Medline and Owens and Minor both manufacture the products they distribute, which means they compete with their own suppliers and control whether a competing product reaches the hospital shelf at all. That conflict is structural, it is openly acknowledged by everyone involved, and it is the single most consequential relationship on the commodity half of this report.
OEM Landscape
The device OEMs that matter on this lens are not the ones that matter on the polymers lens, and the reason is that most of this material never touches a device company.
Becton Dickinson is the largest buyer in the roster by material volume across drapes, procedure packs, prep and the disposables that go with its needle and catheter business. Baxter buys through IV therapy, surgical care and its advanced surgery line. Cardinal Health is the anomaly and the most useful account to understand: it is carried as an OEM because it manufactures its own gowns, drapes, custom procedure packs, gauze and underpads, and it also distributes everything else a hospital buys. A supplier calling on Cardinal is calling on a manufacturer and a channel at the same time, and which conversation is being had depends entirely on which building.
Medtronic and Stryker buy protective and procedural textiles as components of the procedure kits that ship with their capital equipment and their implants, which is a smaller volume than it sounds and a harder specification than it sounds, because the textile is now part of a kit qualified as a system. Teleflex and Integra LifeSciences reach this lens through wound care, tissue products and the drape and prep content of their procedural business.
On the implantable side the customer list is different and much shorter. Medtronic, Edwards Lifesciences, Terumo, Boston Scientific and Abbott buy vascular graft fabric, sewing cuff material, mesh and suture, and they buy it in quantities that would be a rounding error in any other industry in this atlas. What they are actually buying is not fabric, it is a validated construction with a change control commitment and a regulatory file behind it, and the qualification cycle from first sample to commercial volume runs three to seven years. Nobody in that chain switches a supplier to save money.
The buying reality that new entrants most often miss is that the largest volumes in this report are not bought by any of these companies. They are bought by the group purchasing organizations and the integrated delivery networks, through contracts held by Medline, Owens and Minor and Cardinal, and a gown or a drape that is not on one of those contracts effectively does not exist in acute care in North America. The device OEM is the customer for the implantable half and the specification-driven half. The distributor is the customer for the volume half, and the distributor is also the competitor.
One more buyer belongs in this section and rarely appears in a supplier account plan: the federal government. Strategic National Stockpile purchasing, Department of Defense textile procurement under the Berry Amendment, and the Department of Veterans Affairs together buy enough protective apparel to load a domestic line, and they buy it on rules that reward North American manufacture rather than on a group purchasing contract. For a supplier with domestic capacity this is the one channel where the capacity itself is the qualification.
