Electrospun and phase-inversion polyurethane grafts are the standing attempt to solve small diameter patency, which polyester and expanded PTFE both fail at below about six millimetres because the graft thromboses before it heals. Polycarbonate urethane chemistry is more biostable than the polyester urethanes that failed in the 1980s, and the electrospun structures can be built with a fiber scale and porosity that encourages endothelialization. It has been emerging for two decades, which is its own kind of information about how hard the problem is.

The backing has to be transparent so the insertion site can be inspected without disturbing it, waterproof so the patient can shower, and vapour-permeable so the skin underneath survives seven days of occlusion. That is a cast polyurethane film and nothing else, and the moisture vapour transmission rate is on the specification.

The backing decides whether the adhesive gets its fourteen days. A cast polyurethane film with a high moisture vapour transmission rate lets perspiration escape rather than pooling and lifting the patch, and it stretches with the skin instead of levering against it. Choosing a cheaper, less breathable backing is the most common way a sensor patch that passed bench testing fails in a hot-weather field trial.

A transparent polyurethane film backing is what makes a film dressing a film dressing: waterproof and bacteria-proof outward, vapour-permeable so the skin underneath does not macerate, and clear so the wound can be inspected without removing it.
